A parasite outbreak is making headlines, and it is easy to feel confused about what is actually going on.

Is it from lettuce? Salad kits? A restaurant? Imported produce? Contaminated water? And is this the same parasite people talk about in swimming pools?

The short answer: the current concern is Cyclospora, not Cryptosporidium. While both are microscopic parasites that can cause severe diarrhea, they are different organisms with different transmission patterns and treatments.

As of the CDC update on July 21st, 4,173 laboratory-confirmed, domestically acquired cases have been reported across 41 states, including 141 hospitalizations and no deaths.

CDC is also reviewing more than 7,400 additional reports as of July 22nd, and officials expect case counts to rise as more illnesses are confirmed. 

The state of Michigan reported 7,171 cases as of July 22nd. It's important to realize that the CDC and state's numbers may not add up to the same thing, given that CDC numbers can lag behind state numbers. 

Currently, only 9 states have not reported cases: Hawaii, Delaware, Maine, Montana, New Mexico, Oregon, South Carolina, South Dakota, and Vermont. Although that's subject to change also. 

Investigators initially linked the large multi-state outbreak to shredded iceberg lettuce served at Taco Bell restaurants in five states, tracing it to Taylor Farms. However, the FDA has since walked back that finding, saying it was a false positive.

However, health officials caution that not every Cyclospora case is linked to this outbreak. CDC and FDA continue investigating several additional outbreaks, and some sources have not yet been identified.

The good news is there is no reason to panic or avoid all fresh produce.

But it is a good time to understand what Cyclospora is, how it spreads, what symptoms to watch for, and how to reduce your personal risk—especially when eating raw fruits, vegetables, herbs, and leafy greens. 

What Is Cyclospora?

Cyclospora cayetanensis is a microscopic parasite that causes cyclosporiasis, an intestinal illness often linked to fecal-contaminated food or water.

It commonly causes explosive watery diarrhea, fatigue, bloating, appetite loss, stomach cramps, nausea, and sometimes weight loss.

Cyclospora infects the small intestine. It is too small to see on produce, and contaminated food usually looks, smells, and tastes normal.

That is one reason outbreaks can be exasperatingly difficult to trace. You cannot inspect a salad, berry, herb, or vegetable and know whether Cyclospora is present.

In the United States, these foodborne outbreaks have been linked to fresh produce. But it can also be linked to water.

In a past case, the FDA traced a Cyclospora outbreak linked to red cabbage grown in South Florida to water in canals, where the parasite was “readily prevalent” in the water used to irrigate area crops.

Cyclospora Resistant to Treatment

Cyclospora is resilient and is not killed by chlorine, which is the primary disinfectant used in municipal water supplies, according to microbiology researcher and former USDA official Dr. Robert Mandrell.

It has a tough outer shell that lets it survive conditions that kill many other pathogens.

For this reason, some people are calling for an investigation of water systems and not just food in this recent outbreak... especially given the recent heavy rains leading to flooding across large areas of the U.S.

Flooding can cause agricultural runoff and sewage overflows, which could contaminate water used for growing produce.

What Are the Symptoms of Cyclosporiasis?

Cyclospora is best known for severe diarrhea that can be frequent, prolonged, and exhausting.

Common symptoms include:

  • Watery diarrhea.

  • Frequent bowel movements.

  • Loss of appetite.

  • Weight loss.

  • Bloating.

  • Gas.

  • Stomach cramps.

  • Nausea.

  • Fatigue.

Other symptoms can include vomiting, body aches, low-grade fever, or flu-like symptoms.

CDC notes that if untreated, illness may last from a few days to a month or longer, and symptoms may seem to improve and then relapse.

That “relapsing” pattern is one reason people should contact a healthcare provider if watery diarrhea persists for more than a few days, especially during an outbreak.

This pattern is also one of the reasons people don’t initially realize what’s going on. They feel better for a day or two, assume it was a normal stomach bug, and then suddenly relapse.

How Long After Exposure Do Symptoms Start?

Cyclospora symptoms usually begin about one week after exposure, but they can appear anywhere from 2 days to 2 weeks or more after a person eats or drinks something contaminated.

This delay makes outbreaks hard to investigate. If you get sick today, the contaminated food probably wasn’t something you ate yesterday. It may have been something you ate two weeks ago. Do you remember what you ate for dinner two weeks ago? Probably not.

CDC says the incubation period averages one week but can range from as little as 2 days to 2 weeks or more. That’s one reason public health investigators ask detailed food-history questions.

They’re trying to spot patterns across hundreds or thousands of people who may have eaten different foods in different places over a long window of time. 

How Does Cyclospora Spread?

Cyclospora spreads when people consume food or water contaminated with the parasite.

In the U.S., outbreaks have often been associated with fresh produce. That does not mean produce is bad. Fruits, vegetables, herbs, and leafy greens are essential parts of a healthy diet.

But raw produce can become contaminated with bad water in the field, during harvest, processing, packing, transportation, or preparation.

Potential produce categories that have been involved in past Cyclospora concerns include:

  • Leafy greens.

  • Fresh herbs.

  • Berries.

  • Salad mixes.

  • Raw vegetables.

  • Imported produce.

  • Green onions.

Cyclospora is different from many germs because it is not usually spread directly from one person to another.

CDC explains that Cyclospora oocysts shed in stool must mature outside the body in the environment before they become infectious. This process is thought to require at least 1–2 weeks under favorable conditions.

Therefore, direct person-to-person fecal-oral transmission is considered unlikely. That means the bigger prevention focus is contaminated food or water—not casual contact with someone who is sick.

What Do We Know About the Current Outbreak?

Public health officials thought they'd identified a major source of the current Cyclospora outbreak, but perhaps that was too soon, as they've walked back that stance.

As of the latest CDC update, 1,645 laboratory-confirmed, domestically acquired cases have been reported across 34 states, resulting in 141 hospitalizations and no deaths.

CDC is also reviewing more than 5,100 additional reports, and officials expect confirmed case counts to continue increasing as investigations progress.

CDC and FDA had linked the outbreak to shredded iceberg lettuce served at Taco Bell restaurants in Indiana, Kentucky, Michigan, Ohio, and West Virginia.

FDA's traceback investigation identified Taylor Farms de Mexico as the supplier, and the company has voluntarily recalled all iceberg lettuce sourced from central Mexico because of possible Cyclospora contamination.

Taylor Farms also sold the lettuce at retail in WalMart in 27 states, with the brand name Marketside, with "use-by" dates July 18th to August 3rd. If you have it, do not eat it. Dispose of it or return it to the store for a refund. 

However, health officials emphasize that not every Cyclospora illness is linked to this outbreak. CDC and FDA continue investigating additional clusters of illness, and some sources have not yet been identified.

The takeaway is simple: follow official recall notices and public health guidance, but don't assume every salad, restaurant, or produce item is contaminated.

While current evidence seems to point to tainted lettuce, other investigations remain ongoing.

Can Washing Produce Remove Cyclospora?

Washing produce under running water may reduce dirt and some germs, but it may not completely remove Cyclospora due to its resilience and tough shell.

During an outbreak, higher-risk people may want to choose cooked, peeled, frozen, or thoroughly scrubbed produce when possible.

This is one of the most frustrating parts of Cyclospora prevention.

Washing produce is still important. It can reduce dirt, debris, some surface microbes, and some pesticide residues. But it should not be presented as a guarantee.

  • Rub soft produce gently with clean hands.

  • Scrub firm produce with a clean vegetable brush.

  • Rinse produce before peeling or cutting so the knife does not carry contaminants into the flesh.

  • Remove damaged or bruised areas.

  • Discard outer leaves of head lettuce and cabbage, where parasites are more likely to reside.

  • Dry produce with a clean towel or paper towel.

  • Keep produce separate from raw meat, poultry, seafood, dirty cutting boards, and dirty sink surfaces.

  • Clean/disinfect sink and countertops before bringing produce out.

  • Refrigerate cut, peeled, or cooked produce promptly if not eating immediately.

This matters even for produce you plan to peel. If you slice through an unwashed melon, cucumber, apple, or avocado, the knife can drag surface contamination into the edible part.

Firm produce is easier to clean well because you can scrub it. Delicate produce—such as berries, herbs, lettuce, and leafy greens—is harder because you cannot scrub aggressively without damaging it.

It's a good idea to get in the habit of washing your fruits and vegetables this way, even once this outbreak passes. The CDC notes that Cyclospora sicken people every year. It's just that the scope of damage is much higher right now.

What About Hydrogen Peroxide, Bleach, Vinegar, or Baking Soda?

This is where online advice can become confusing—and potentially unsafe. While these may sound plausible, they are unproven.

Hydrogen Peroxide:

Hydrogen peroxide may have roles in certain controlled food-industry or surface-disinfection settings but is not recommended as a home produce wash.

Even “food-grade” hydrogen peroxide can be highly concentrated and dangerous if mishandled.

Food-industry sanitation uses very carefully controlled concentrations, contact times, equipment, and rinsing procedures. That is not the same as pouring hydrogen peroxide into a kitchen bowl and soaking salad greens.

Bleach:

Do not wash fruits and vegetables with bleach or household disinfectants. Produce is often porous, and chemical residues can remain on food. Bleach is appropriate for surface disinfection, not home produce washing.

Vinegar:

Vinegar may help remove some dirt or residue, but it should not be relied on to remove or kill Cyclospora. It is an unproven parasite-control step.

Baking Soda:

Baking soda may help reduce some surface pesticide residues on firm produce when used as a soak and followed by thorough rinsing.

But baking soda is not a proven parasite-kill step. It does not sterilize produce, and it should not be considered a Cyclospora solution.

Commercial Produce Washes:

Commercial produce washes are not necessary for most consumers and should not be assumed to remove Cyclospora.

Plain running water, friction, scrubbing firm produce, drying, safe storage, and cooking when appropriate remain the more responsible consumer guidance.

When Cooking Is the Safer Choice

Washing has its limits. Cooking is more reliable than washing alone because heat can kill many foodborne pathogens.

During an active outbreak like what’s going on right now, or for people at higher risk, it may be wise to choose more cooked produce and fewer raw high-risk items, at least for the duration of the outbreak.

Consider cooked options such as:

  • Steamed greens.

  • Roasted vegetables.

  • Sautéed vegetables.

  • Soups and stews.

  • Cooked peppers, onions, mushrooms, squash, carrots, and broccoli.

  • Frozen vegetables cooked according to directions.

  • Canned vegetables from reputable sources.

Consider extra caution with:

  • Raw leafy greens.

  • Pre-cut salad mixes.

  • Fresh herbs.

  • Raw garnishes.

  • Berries.

  • Uncooked vegetables served in large batches.

  • Produce from unknown or untraceable sources.

This does not mean you should stop eating produce. It means you can shift toward lower-risk options when the situation calls for it.

Who Is at Higher Risk?

Cyclospora can make anyone sick. But some people should be more cautious because diarrhea and dehydration can become serious faster.

Higher-risk groups include:

  • Older adults.

  • Young children.

  • Pregnant individuals.

  • People with weakened immune systems.

  • People taking immune-suppressing medications.

  • People undergoing cancer treatments.

  • People with HIV/AIDS.

  • Organ-transplant recipients.

  • People with chronic illness.

  • People who dehydrate easily.

If someone in a high-risk group develops severe diarrhea, they should contact a healthcare provider promptly.

How Is Cyclosporiasis Diagnosed and Treated?

Cyclosporiasis is diagnosed with stool testing, but Cyclospora testing is not always included in routine lab panels. CDC says healthcare providers may need to specifically request testing for Cyclospora.

This is important. If someone has watery diarrhea for days or weeks and a basic stool test is negative, that doesn’t necessarily rule out Cyclospora.

CDC says testing for Cyclospora is not routinely conducted in most U.S. laboratories, and not all gastrointestinal PCR panels include Cyclospora. Healthcare providers may need to specifically request Cyclospora testing.

The standard treatment is also different from many other stomach bugs. CDC says trimethoprim-sulfamethoxazole, also called TMP-SMX, Bactrim, Septra, or Cotrim, is the treatment of choice for cyclosporiasis.

CDC also notes that most healthy people eventually recover without treatment, although the illness may be prolonged, and no vaccine is available.

People who are allergic to sulfa drugs should not self-treat. They need medical guidance, because CDC notes that no highly effective alternatives have been identified yet for people who cannot take TMP-SMX.

When Should You Call a Doctor?

Call a healthcare provider if you have:

  • Watery or severe diarrhea lasting more than 2–3 days, or if it quits and then relapses.

  • Diarrhea that comes and goes for weeks.

  • Signs of dehydration.

  • Severe fatigue.

  • Significant weight loss.

  • Fever or worsening abdominal pain.

  • Blood in the stool.

  • Recent exposure to raw produce linked to an outbreak.

  • Recent travel to a high-risk area.

Ask specifically whether Cyclospora testing is appropriate, especially during an active outbreak.

How Produce Cleaning Also Helps Reduce Pesticides and Mold Exposure

Produce safety is not just about parasites.

Proper produce washing helps reduce dirt, some surface microbes, insects, garden debris, some pesticide residues, and mold spores. But it does not remove every pesticide or make spoiled produce safe.

For pesticide residues, running water plus rubbing or scrubbing is usually more helpful than a quick dunk.

A baking soda soak may reduce certain surface pesticide residues on firm produce, but it does not remove all pesticides and does not address residues absorbed into the plant.

For mold, do not rely on washing. Moldy soft produce should be discarded. That includes berries, peaches, tomatoes, cucumbers, leafy greens, and other high-moisture foods.

Don’t fear eating fruits and vegetables. Just make your produce habits smarter during an outbreak.

Here are some practical steps:

  • Choose whole produce over pre-cut or bags when possible. Get a whole head of lettuce or cabbage, discard the more-exposed outer leaves, and thoroughly clean the rest of the leaves under running water, rubbing each leaf. 

  • Wash produce right before use, not days ahead, unless it needs to be dried and stored properly.

  • Consider produce from a local farmer's market, which is less likely to be contaminated, according to USA Today. Home garden produce is also more likely to be safe. 

  • Scrub firm produce well, with a clean brush.

  • Choose cooked vegetables during outbreak concerns.

  • Use peelable produce when risk seems higher.

  • Avoid visibly damaged, slimy, or moldy produce.

  • Keep raw produce away from raw meat and dirty sink areas.

  • Wash reusable produce bags regularly.

  • Clean cutting boards and knives after each use.

  • Follow recall and outbreak alerts.

  • Do not panic.

  • Don’t assume one food is responsible until officials confirm it.

  • Don’t rely on produce-washing hacks that are unproven for Cyclospora.

  • Do wash produce carefully and thoroughly under running water.

  • Do choose cooked, peeled, or scrubbed produce when risk is higher.

  • Do call a healthcare provider for persistent watery diarrhea.

  • Do ask about Cyclospora testing if ill.

Cyclospora may be microscopic, but the prevention habits are practical.

Wash your hands. Handle produce carefully. Store food safely. Stay alert to public health updates.

And during an outbreak, remember that a cooked vegetable may be the safer choice than a raw salad — at least until investigators have clearer answers.
